• PLEASE BOOK THE APPOINTMENT IN YOUR BABY’S NAME (Not the parents name).
• ATTENDING CLINIC WITH YOUR BABY – Maximum of 2 Adults to accompany. Please arrange for other siblings to be cared for elsewhere during the appointment.
